    Necessary Documents for Application of Certificate of Origin Form - D

    * Application Form with the company’s Letter Head
    * Form-D duly filled completely by English typewriter
    * Copy of Bill of Lading
    * Copy of Relevant Export License
    * Copy of Packing List
    * Copy of Invoice
    * The Statement showing the application of Form-D in conformity with the export license granted
    * The letter of undertaking shall be submitted for wholly obtained products
    * The following supporting documents shall be submitted for manufactured and semimanufactured products
    (a) Cost Sheet (as per cost sheet, products with import contents not more than 40%).
    (b) For export products under CMP system, the undertaking for availability of the local expenses shall be submitted.
